Fire Incident Report FORM

You can request a copy of Collierville Fire Incident Report by either contacting the Fire Records Administrator or submitting your request electronically with the form below

  • The fee for a Fire Incident Report is $10; cash, check or money order, payable to the town of Collierville.

Be prepared to provide the following information:

  • Incident Date and Time

  • Incident Location (street address, intersection, etc.)

  • Incident Type (structure fire, vehicle fire, etc.)

  • Your contact information (name, email, phone number, fax number, and mailing address). 

Once notified, reports may be paid for and picked up in person at Fire Administration, 1251 Peterson Lake Road, Collierville, TN 38017.

    • We will make every effort to respond to your request within 15 business days, but remember incidents under investigation are not available until cause/determination has been made, which could take +/- 30 days. Once we notify you, reports may be paid for and picked up in person at Fire Administration, 1251 Peterson Lake Road, Collierville, TN 38017
